How to customize your registration form

  1. In the app, go to Forms and select the form you want to edit (or create a new one).
  2. Use the Fields tab to add, remove, and reorder the customer fields your applicants will fill in — choose from 20+ field types including text, dropdowns, checkboxes, and file uploads.
  3. Switch to the Content tab to edit the form’s title and description shown to customers.
  4. Use the Confirmation tab to set what applicants see after submitting — a custom message or a redirect to another page.
  5. On paid plans, the Email tab lets you customize the notification emails sent for new applications, approvals, and rejections.
  6. Use the Appearance tab to match your form’s colors, fonts, spacing, and layout to your store’s branding — no code required.
  7. Changes save automatically as you edit.

How to add the form to your store

  1. From the Forms page, click Add theme block (or open your theme editor directly).
  2. In the Shopify theme editor, add the Wholesale Registration app block to any page — commonly the homepage or a dedicated “Become a Wholesale Buyer” page.
  3. Preview how the form looks on your live storefront before publishing.
  4. Publish your theme changes to make the form live for customers.

How to review and approve applications

  1. Go to Applications to see every submission across all your forms, with filters for status (Pending, Approved, Rejected), form, and date.
  2. Click any application to view the applicant’s full submitted details.
  3. Add an internal note (visible only to your team) to record why you’re approving or rejecting an application.
  4. Click Approve to automatically create or update the customer’s Shopify customer record — tagged and ready for wholesale pricing — or click Reject to decline the application.
  5. Approved customers are synced instantly, with no manual data entry required.
